Preheat the oven to 350˚F (175˚C). Line a loaf pan with parchment paper and set aside.

In a small bowl, combine brown sugar, cinnamon, and nutmeg. Mix thoroughly and set aside.

In a large bowl, combine granulated sugar, melted butter, sour cream, eggs, milk, and vanilla extract. Mix by hand or with an electric mixer until smooth and combined. In a separate bowl, whisk together the flour, baking powder, baking soda, and salt. Add dry ingredients to the wet mixture and mix until just combined, being careful not to overmix.

Peel, core, and chop the Granny Smith apple into 1/2-inch cubes. Reserve a handful of chopped apples for topping.

Pour half the batter into the prepared loaf pan and smooth into an even layer. Sprinkle half of the cinnamon sugar filling over the batter and gently swirl with a butter knife. Repeat with the remaining batter, reserved apple pieces, and the rest of the filling, swirling the top layer with the knife.

Bake in the preheated oven for 50-55 minutes, or until a toothpick inserted into the center comes out clean. Allow to cool on a wire rack for 10-20 minutes.

In a small bowl, whisk sifted icing sugar with hot water until the desired consistency is achieved. Spoon or drizzle the glaze over the cooled loaf.

Allow the glaze to set for 5-10 minutes before slicing the loaf and serving.
